Placebos are commonly understood to be inert treatments, such as sugar pills, that have no pharmacological effect, but the AMA defines placebos more narrowly, as therapies that a physician believes lack a specific pharmacological effect on the conditions being treated. The physician's belief may or may not align with the prevailing medical view of a treatment. "There are borderline and controversial cases, such as acupuncture and antidepressants, in which individual physicians might reasonably disagree with the medical community's consensus about whether a treatment is an active treatment or a placebo," writes Anne Barnhill, a philosopher and bioethicist who is studying social work at Columbia University.

The article cites a recent poll of American internists and rheumatologists that found that a significant number of them admit to giving patients placebos without disclosing the therapies as such. While some placebo use is patently unethical providing a treatment that "has no scientific basis and is dangerous, is calculated to deceive the patient by giving false hope, or which may cause the patient to delay in seeking proper care" other uses of placebos are widely seen as ethical, writes Barnhill.

Some placebos might offer medical benefit to patients with certain conditions, Barnhill notes, and the limited available data suggest that placebos are more effective when presented as active treatments. As a result, she adds, some bioethicists have argued that an undisclosed placebo is the best available treatment for some patients. "If the best available treatment is sometimes an undisclosed placebo," she writes, "then the AMA's policy prohibits physicians from offering the best available treatment in some cases."

In addition to failing to benefit patients, the AMA policy may not meet two of its other goals: protecting patients' autonomy and their trust in physicians. The rationale for requiring physicians to disclose their belief that a treatment is a placebo is that patients need this information in order to give informed consent about whether to take the treatment. Informed consent is essential to patient autonomy. But it is unknown whether patients find this information relevant to their decision-making, Barnhill says, because "there's little data on patients' attitudes toward placebos."

Because of this lack of data, Barnhill also argues that the AMA policy does not help protect patients' trust in physicians. "The AMA seems to assume that uncovering undisclosed placebo use reduces patients' trust in physicians. But this is not a given," she writes. "When they uncover undisclosed placebo use, patients might conclude that their physicians are untrustworthy liars or quacks, or that their physicians do not believe that they are truly sick or, that their physicians are open-minded, cutting-edge, and savvy about mind-body connections."

Barnhill recommends that the AMA consider revising its policy on placebo use. If the goal is to protect patients from harm, safeguard their trust, and respect their autonomy, she says, then rather than requiring physicians to disclose their personal belief about whether a treatment is a placebo, the policy might require physicians to report on the medical community's consensus on the treatment's status.

In the same issue of the Hastings Center Report is a commentary by Bette-Jane Crigger, director of Ethics Policy for the AMA and secretary of the Council on Ethical and Judicial Affairs, which wrote its ethical guidelines on placebo use, and Matthew K. Wynia, director of the AMA's Institute of Ethics. Regarding Barnhill's recommendation that the placebo policy be based on professional consensus, rather than individual doctor's judgment, they write, "We'd be tempted to agree but, as in so much of medicine, it isn't clear that a strong consensus is actually possible here." How, they ask, should doctors distinguish between so-called impure placebos medications that have a pharmacological effect on some illnesses but not necessarily for the ones for which they are being prescribed from off-label prescribing?

Crigger and Wynia emphasize that the overarching intent of AMA policy is to encourage physicians to be honest with their patients. "If there is professional disagreement on how or whether a particular pharmacologic agent works, then patients deserve to know that," they write. "If a doctor holds an outlier view, then his or her patients deserve to know that as well.

?We?re excited that NFL Films and HBO has chosen the Miami Dolphins to appear on Hard Knocks,? Philbin said, via the video box on the right rail of the site.? ?I want you to know a couple things about the decision.? Number one, it was a football decision.? It was made by the football operations staff.? Our owner, [Stephen Ross], supported our decision but in no way, shape, or form forced us to make this decision.?

Many will believe otherwise, especially since G.M. Jeff Ireland grew up in the Bill Parcells school of ultra-secrecy.

But as the Dolphins struggle to generate interest in the team via conventional means (you know, by winning more games than they lose), the team has to do something to get people sufficiently engaged to buy tickets to the games.? Thus, the Dolphins are now more available to the media ? and they?ll be even more available once the NFL Films cameras and microphones show up.

To the extent that the appearance arises from a football decision, Philbin didn?t really articulate a football reason.? He said the that show will ?showcase our players? and provide a ?great opportunity for us to communicate with our fans.?? While those concerns reflect prudent business reasons, there?s no obvious basis for the move, from a football standpoint.

In contrast, Vince Lombardi liked having NFL Films camera crews around because it made his players work harder.? If that?s the basis for the ?football decision,? why not say so?

And so the Dolphins will deny that the move was driven by dollars and cents, and the rest of us won?t believe them.? But at least we finally know who the team will be, even if the end result likely will be far less compelling than if the Jets, Saints, Broncos, Colts, or Redskins had done it.

Imagine if E.T. phoned Earth and heard, ?We?re sorry, no one is here to take your call right now. Please try again later.? Or if, after traveling for thousands of years, radio signals carrying the imprint of an alien technological civilization fell on dormant Earthly ears.

These possibilities could become reality if funding forces the Search for Extraterrestrial Intelligence to turn off the telescopes and quit searching for the answer to one of humankind?s most profound questions.

Jill Tarter isn?t going to let that happen.

After 35 years of eavesdropping on the stars, hoping to hear the intelligent murmurings of an extraterrestrial civilization, the SETI Institute?s lead alien hunter is hanging up her receiver and retiring as director of the Center for SETI Research.

But she?s not quitting the quest.

Instead of querying the heavens for signs of life, Tarter will query life on Earth for the funds needed to keep Earth?s ears online. ?We?re just going to make this a crusade,? she told me during an interview. (You can read it here.)

Physicist Gerry Harp has taken the reins from Tarter, the SETI Institute announced on May 22. ?It?s been challenging and exciting, a lot of new things to learn, a lot of new meetings to go to,? Harp says of his new role. ?But from what I?m seeing, Jill?s working at least as hard as she ever did before. I think we should acknowledge that she is still very much a part of the program.?

Tarter?s curiosity about life on other worlds erupted in the 1970s, fueled by a serendipitous merging of her ability to program a very old computer (a PDP-8/S ? ?I always thought the ?S? stood for ?stupid,?? she says), with a forward-looking proposal to find out whether other communicating civilizations exist in the galaxy. At the time, Tarter was in graduate school at the University of California, Berkeley. ?I couldn?t imagine anything more exciting or rewarding than trying to answer this old question,? Tarter says. ?I got hooked and I stayed hooked.?

In 1984, Tarter and several colleagues founded the SETI Institute, a nonprofit organization that has since grown to include more than a hundred people. Once focused mainly on detecting radio signals from extraterrestrial civilizations, the institute now employs scientists studying many factors important for the existence of alien life ? from Kepler team members who are detecting and characterizing exoplanetary systems, to astrobiologists searching for life in the most extreme environments on Earth and in the solar system?s exotic nooks and crannies.

But the institute needs money to keep searching. Once funded by a fraction of a percentage of NASA?s budget, SETI has relied on private donors to maintain the interstellar dial tone since 1993 ? courtesy of one senator, who killed any substantial SETI funding for the foreseeable future. ?For years, we became the four-letter S-word that you couldn?t actually say at NASA headquarters,? Tarter recalls.

Now, with the Allen Telescope Array ? a SETI array comprising 42 dishes (out of a planned 350) ? that funding is more crucial than ever. Last April, the array went into hibernation after funds dried up. In October, the array returned to operations, propelled by individual donors who contributed more than $200,000. Now, the telescope is under new management by SRI International, and is supported by a contract from the U.S. Air Force that tasks it with searching for orbiting space trash.

Still, 50 percent of the observing time is occupied with SETI searches. ?For SETI, it is the best thing around,? Harp says. ?We have 12 hours a day on this telescope. It?s very difficult to get 12 hours a day on any telescope in the world, and that?s what we have.?

Tarter is hoping to once again tap the enthusiasm of SETI supporters and build a lifeline that will support the search for centuries. ?SETI really is the investment in our future,? she said. ?If we detect a signal, we know we can have a long lifetime.? There will be proof that it is possible to become an old technology. Someone else did it. Therefore, damn it, we can do it.?

Coming in second on the list was Kahanamoku Beach in Waikiki on the island of Oahu in Hawaii. The Dr. Beach website describes the destination as benefiting from a shallow offshore reef that protects swimmers from big waves, making it ideal for children.

A church video surveillance system will offer members a secure and safe place to worship God with other believers especially for those who see it as a growing necessity in today's world.

Another concern in today's society is the prevalent need to keep children safe. Unfortunately with divorce rates up children are at a greater risk for being victims. A church security system can provide attendance tracking and child security for every member of a family. Self check in technology can make it easy and flexible for each member attending services. One person can go to a computer screen and sign in the entire family. Before a person can pick up a child they have to be able to use the check in system with security code. This eliminates the possibility of someone who is not authorized to pick up the child.

High tech equipment is able to use surveillance that includes enhanced night vision and tracking features. Motion detectors allow the church video surveillance system to target in on any motion and follow it as long as it is detected. Equipment that is tied in with a computer assisted operation can allow the operator to set the controls to high alert so that an alarm will sound in case anything or anyone moves in front of the view of the camera. Some systems that are much more expensive can even detect motion behind walls. Others include infrared technology that picks up on anything that gives an energy signature so that anything can be detected in spite of bad weather or other types of interference.

Some high tech cameras for surveillance have a protective casing to protect them from vandalism and some are very small and can be hidden from sight. Technology in this area has become so advanced that the computer can be programmed to zoom in and scan items that may be hidden under clothing such as guns or other objects that can be harmful to others. This type of technology is already being used by some law enforcement and government officials. In some areas cameras on toll roads can take a picture of a car and bill that person for toll fees. Cameras stationed at traffic lights can take a picture of a car that has run a red light and bill the owner for a traffic violation.

Real Salt Lake still rolling

SANDY, Utah (AP) - Nat Borchers doesn't score many goals. When the veteran defender does come up with one, however, they usually are clutch.

That was certainly the case Saturday night when Borchers scored in stoppage time to give Real Salt Lake a 3-2 victory over FC Dallas.

Alvaro Saborio had two second-half goals to help Salt Lake (9-3-2) extend its unbeaten streak to six games. Dallas (3-8-4) is winless in nine games, despite getting second-half goals from Matt Hedges and Blas Perez, and hasn't won in Utah since 2007.

Borchers scored in the 93rd minute. A loose ball fell to his feet deep in the center of the box after being knocked around a bit. Borchers had a clear lane for a shot and the veteran defender hooked the ball inside the right post.

"We were pushing for that goal," Borchers said. "All the guys were fighting and were getting chances. It's going to happen when you put pressure on a team like that."

Injuries and absences for other reasons left both teams short-handed. A total of seven midfielders and defenders were scrubbed because of injuries for FC Dallas. An eighth player, Brek Shea, wasn't available because of a suspension. RSL didn't have goalkeeper Nick Rimando or team captain Kyle Beckerman because both were playing with the U.S. national team Saturday.

Even while suffering a loss, Dallas coach Schellas Hyndman felt encouraged by the effort he saw for 90 minutes. Hyndman was convinced his team did enough to nearly steal a point on the road.

"It's encouraging to score two goals," Hyndman said. "It's more encouraging to come back twice and not give up with the adversity that was out there."

Effects from the talent drain were felt in the first half. Dallas and Salt Lake combined for just seven shots and neither team could do much to gain advantage. Windy conditions also made balls hang in the air longer than normal and caused problems for each offense.

RSL coach Jason Kreis counted on things changing once the conditions grew more manageable in the second half.

"I wasn't anticipating the gates to open against us, but I was anticipating for us to score some goals - which we did," Kreis said.

Salt Lake finally found some life on offense in the 59th minute. Terakazu Tanaka sent a cross to Saborio at the right post. Saborio leapt up and sent a header through the outstretched hands of goalkeeper Kevin Hartman.

Dallas tied it at 1 in the 75th minute when Hedges launched a right footer from a difficult angle and snapped a 325 minute shutout streak by RSL. Goalkeeper Kyle Reynish actually parried away the initial shot from Hedges, but he did so from behind the goal line and the goal counted.

Saborio came through right away to extinguish any momentum Dallas gained from the equalizer. He latched onto a pass from Ned Grabavoy and then drilled a right footer from the right side of the box to make it 2-1 for Salt Lake.

Dallas tied it at 2 in the 85th minute when Perez fired a shot from the center of the box and the ball slipped off Reynish's glove.

Borchers erased those rallies in the end, but losing the lead twice did not sit well with several players in the RSL locker room.

"When you're out there and you give up leads like that, it's just absolutely deflating - especially when you work that long in the game to not really give them anything," Grabavoy said.

UNCASVILLE, Conn. (AP) ' Becky Hammon hopes that the Olympics will be a little less stressful for her.

Hammon will once again play for the Russian women's basketball team at the games this summer in London. She created a stir in 2008 when she first played for Russia in the Beijing Olympics. She helped guide that team to a bronze medal.

"It's old news," Hammon about her decision to play for Russia. "I played with them in the world championship, European championship, hopefully I'll enjoy it a lot more. By far it was one of the best decisions I ever made and I stand by that, and really look forward to doing it again."



When Hammon, who was raised in South Dakota and played to Colorado State, initially decided to play for Russia in 2008 it ignited a media firestorm with people questioning her patriotism, including 2008 U.S. Olympic coach Anne Donovan.

Every time Hammon went on the road with the San Antonio Silver Stars she had reporters waiting to talk to her about her decision to play for Russia.

It got to be a bit much.

"I had reporters from all over the world asking me questions," Hammon said. "Everywhere I went that's all anyone wanted to talk about. It's nice that this time people seem to be focused on other things. It seems almost every team now has one or two players with foreign passports playing for them. It goes back to the whole 'it's Russia'. If it was a country no one heard of, no one really would have cared. That's the society we live in."

Now no one seems to notice anymore.

"It's played through once," Silver Stars coach Dan Hughes said. "The reality of it is we've been through it one time already and Becky is an example of what I think a lot of teams that the U.S. will face. There are a lot of players in our league that are now on other teams and going to be part of the Olympic experience."

The 35-year-old point guard plays for a Russian club team during the winter and became a naturalized citizen. Because she hadn't played for the United States in any major FIBA-sanctioned international events, she was allowed to compete for Russia in the Beijing Olympics.

While Hammon is happy that the firestorm seems to have died down this time around, she also is proud that her Russian has improved over the past few years.

"I understand a little bit, it's a lot better than 2008, put it that way," she said laughing.

While Hammon had to miss a few games for the Silver Stars in the past to train with Russia, she won't join the national team until the WNBA takes its month-long break in the middle of July.

"I won't be missing any Silver Stars games and that's important to me," Hammon said.

Hammon is excited to once again march in the opening ceremonies and take part in the games.

"It's the Olympics, it should be fun a great time I think. It will be my first time in London for any length of time," Hammon said. "I'm looking forward to be in London. Get back together with the girls from the team, hopefully make a great run."

One player that Hammon won't be playing with this time around is center Maria Stepanova. The 6-foot-8 star, who has played in the last four Olympics, tore her anterior cruciate ligament at the Euroleague final eight in late March.

"She's been the face of the franchise, so that's a tough loss for us," Hammon said. "There's some young kids that are very talented post players coming up, too. Someone of her caliber you're going to miss, no way around that. But it's also another opportunity for someone else to step up and prove that they belong at the highest level."

She hopes that even without Stepanova, the Russians can make another run at a medal. They are in the same pool as Australia and wouldn't have to face the heavily-favored U.S. team until the elimination rounds.

"At the end of the day it's been a really cool journey and unique journey no one has ever done," she said.

Mexico's Michel Franco wins Cannes sidebar prize

(AP) ? "After Lucia" by Mexican director Michel Franco has won the top prize in the Cannes Film Festival's sidebar competition, Un Certain Regard.

It was chosen Saturday from a slate of 20 films by a jury headed by British actor Tim Roth.

Un Certain Regard focuses on new and emerging filmmakers.

The jury gave second prize to French film "Le Grand Soir" by Benoit Delepine and Gustave Kervern, and a "special distinction" citation to Bosnian film "Djeca" ("Children of Sarajevo") by Aida Begic.

Twenty-two films from around the world are competing for prizes in the festival's main competition. Winners will be announced Sunday.

Favorites for the top prize, the Palme d'Or, include Michael Haneke's "Amour" and Christian Mungiu's "Beyond the Hills."

Devils make Cup finals after 3-2 win over Rangers

NEWARK, N.J. (AP) ? Moments after sending the New Jersey Devils to the Stanley Cup finals for the first time since 2003, Adam Henrique skated to the corner and waited to be mobbed by his teammates.

It's become a common occurrence for the 22-year-old who spent last season in the minor leagues. He notched the series winner in double overtime against Florida in Game 7 in the opening round, and did it again on Friday night in Game 6 of the Eastern Conference finals, sending the rival New York Rangers packing for the season along the way.

And, oh, how sweet it was for the Devils.

The memory of missing the playoffs last season and the ghost of an 18-year-old wound were wiped out with a sweep of this rolling rookie's stick.

Henrique scored off a wild scramble in front at 1:03 into overtime and the Devils eliminated the Rangers 3-2 to advance to the Cup finals against the Los Angeles Kings.

"I mean, this kid's just at the right place, right time, all the time," Devils coach Peter DeBoer said. "The two biggest goals of the playoffs come off his stick.

"And that's not accidental."

Both goals have been followed by scrums in which teammates surround Henrique and pound on him to celebrate. It's fun, of course, but Henrique is more focused on facing the Kings in a series that will start in New Jersey on Wednesday.

"There's still more things to accomplish," said Henrique, who is a finalist for the Calder Trophy, given to the NHL's top rookie.

"Try to take in as much as I can this whole year. And obviously those are two huge goals, and then probably two of the biggest goals I ever scored. There's still more ahead. We're not done yet. We haven't really won anything. It's a long ride.

"Every kid dreams of playing in the Stanley Cup final. And we have a chance to win. I'm excited, and ready to get started."

After beating Florida, Philadelphia and their biggest rivals, the Rangers, only the Kings stand in the way of a fourth Cup for New Jersey.

"It feels great to get to where we are," Devils captain Zach Parise said. "As Adam said, we still have a lot of work ahead of us. But we've put ourselves in a pretty good position right now, and we've got a chance.

"We'll see what we can do."

This series win came against the Devils' most intense rival, and it was that much more enjoyable ? even to those who haven't been around the feud all that long.

"That one was like Christmas," Henrique said.

It also was needed. The Devils, after all, blew a 2-0, first-period lead and didn't want to head back to New York for a Game 7 on Sunday.

"It didn't matter how it got to overtime, we were in a good position," Parise said. "We were at home. We just needed one shot."

Actually, the Devils needed a few shots to win the game.

Henrique's winner came after Henrik Lundqvist stopped Ilya Kovalchuk twice and Alexei Ponikarovsky. The last shot was in the crease and Henrique tapped it home.

"We caught them on a line change and their defensemen were tired," said DeBoer, who was fired by Florida after missing the playoffs in his three seasons with the Panthers.

"We found a way to jam one in. That's the only way you're going to score on Lundqvist. You're not going to get a clean one. You're going to have to work for it around the net.

"And that's what we did."

Ryan Carter and Kovalchuk also scored for the Devils, whose biggest move this year was hiring DeBoer. He has more than proved his coaching ability.

Ruslan Fedotenko and Ryan Callahan tallied for top-seeded New York, which had a good flurry just before New Jersey scored to end it.

The six Rangers on the ice just stayed down in disbelief and frustration. This was very much like Game 5, which the Devils won 5-3. New York carried the play after the first period and had a 35-29 edge in shots.

But when it came time for a game-deciding play to be made, it was a Devil who made it.

"When they scored, it was such an empty feeling," said Lundqvist, who said the puck took a weird bounce on the final play. "It is shocking."

Henrique overcame injury to score this one. He seemed to take a stick from Brian Boyle in the groin area late in the third and had to leave the ice.

He felt no pain after the game winner.

All the Rangers could do was bow their heads and then line up for the traditional handshake after losing to their cross-rival rivals in a series that was close.

"That's playoff hockey, and that's usually where you get an overtime goal," New York's Brandon Prust said. "Just whacking away in front of the net, getting rebounds."

The ageless Martin Brodeur, 40, kept the Devils alive in the third. He stopped a power-play shot by Brad Richards, made a save on Artem Anisimov between the circles and used his stick to deflect a pass from the boards by Carl Hagelin in the final minute just before it got to Marian Gaborik on the edge of the crease.

"There's a reason why he's the best goalie to play the game," Henrique said. "He gives us that extra confidence every night when we got him back there. He's going to give us the chance to win no matter what, even if we're not playing our best. He seems to make those big saves night in, night out, and he did it again tonight."

Lundqvist's best stop in the third was on Dainius Zubrus on a shot from behind the circles.

Facing elimination and down 2-0 after 20 minutes, the Rangers found their game in the second period and tied it at 2-all on goals by Fedotenko and Callahan in a roughly four-minute span.

Carter, who scored the game winner in New York on Wednesday after the Devils blew a 3-0 lead, put New Jersey ahead again at 10:05 of the opening period on a rebound. Kovalchuk added his seventh goal of the postseason on a power play, his fifth with the extra man.

The Devils ? as is the tradition for many Cup finalists ? did not touch the Prince of Wales Trophy that was presented at center ice. As the team skated off to their locker room, "Glory Days," the 1984 hit from New Jersey rocker Bruce Springsteen serenaded them.

The game was played on the 18-year anniversary of the Rangers' dramatic 4-2 Game 6 victory over New Jersey at the Meadowlands, a win that pushed that classic Eastern Conference final series to a Game 7 and eventually led New York to its first Stanley Cup in 54 years. That game, of course, was preceded by a guarantee from Rangers captain Mark Messier, who delivered three goals en route to the victory.

This time, though, there will be no Game 7.

NOTES: The Rangers became the fifth team to play 20 postseason games without reaching the finals. ... TV personality Donald Trump was at the game. ... Devils C Travis Zajac left the ice briefly in the second after being slashed on the left hand by Prust. No penalty was called. ... New Jersey is 4-1 in overtime in the postseason. New York finished 2-3 after regulation. ... This is only the second time the Devils have defeated the Rangers in the postseason in six tries. ... The last time New Jersey played in the Stanley Cup finals, the Devils also played a California team: the Anaheim Ducks in 2003.
